Women are born with all the eggs they will ever have. By the early 40s, both the quantity and chromosomal quality of remaining eggs have declined significantly.
Chromosomal quality determines whether a fertilised egg develops into a healthy embryo, implants successfully, and results in a live birth. Chromosomally abnormal eggs are the most common reason for failed cycles and early miscarriages in older women. This is not about general fitness or health. A 43-year-old who is physically healthy still has eggs that carry a higher risk of chromosomal abnormality than a 32-year-old with the same profile.

IVF Success Rates After 40: The Honest Numbers
Using one's own eggs, IVF success rates per fresh cycle at a quality centre are as follows:

Can I Try IVF with My Own Eggs After 40?
Yes, and many women do. The key investigation is your AMH (Anti-Mullerian Hormone) level combined with an antral follicle count on ultrasound. Together, these tell your doctor how many eggs remain and how your ovaries are likely to respond to stimulation.
If your AMH is in an acceptable range and your follicle count is reasonable, own egg IVF is a fair starting point. Your doctor at Delhi IVF will give you a direct, honest reading of what your numbers suggest before you commit to a cycle.
If AMH is very low and the follicle count is poor, stimulation may yield very few eggs and increase the risk of cycle cancellation. In these cases, donor egg IVF is often the more practical and emotionally sustainable path from the outset.
What is Donor Egg IVF and How Does It Work?
Donor egg IVF uses eggs from a younger, screened donor. They are fertilised with the partner's sperm, and the embryo is transferred into the intended mother's uterus. The pregnancy is carried entirely by you.
At Delhi IVF, all egg donors are aged between 21 and 30, screened for infectious diseases, genetic conditions, and psychological suitability, and matched to recipients on relevant physical characteristics.
The recipient process is simpler than standard IVF. There is no stimulation phase. You take hormonal medication to prepare your uterine lining, and the embryo is transferred once the lining is ready.
For many women over 40, particularly those over 43, donor egg IVF is not a compromise. It is the most effective treatment available, and the success rates reflect that.
IVF at 42, 43, 44 and Beyond
At 40 to 42: Own egg IVF remains a reasonable option for women with adequate ovarian reserve. Success rates are lower but meaningful enough to justify an attempt if investigations support it.
At 42 to 44: Egg quality declines more sharply. Some patients still achieve success with their own eggs if AMH and follicle count are acceptable. Many find donor egg IVF offers a better chance with less physical and emotional cost.
At 45 and above: The chance of a successful pregnancy with one's own eggs is very low. Donor egg IVF is the clinically recommended path for most patients in this group. Success rates with donor eggs remain comparable to those of younger recipients because egg quality depends on the donor's age, not the recipient's.
Indian law does not set a strict upper age limit for IVF in women who are still menstruating. Post-menopausal IVF with donor eggs has been performed in India, though it requires careful clinical evaluation.

Preimplantation Genetic Testing (PGT) After 40
PGT tests embryos for chromosomal abnormalities before transfer. Only chromosomally normal embryos are transferred, which improves implantation rates and significantly reduces miscarriage risk. For women over 38 using their own eggs, it is a conversation worth having with your doctor.
The trade-off is that abnormal embryos are excluded, reducing the number available for transfer. In cycles that produce few embryos, this needs to be weighed carefully. Your doctor at Delhi IVF will advise based on your age, embryo numbers, and history.
Lifestyle Factors That Support IVF After 40
Egg quality is largely determined by age and genetics. These steps will not reverse the biological timeline, but they put your body in the best possible state for treatment.
Maintain a healthy weight: A BMI between 19 and 30 supports hormonal balance and ovarian response.
Stop smoking: Smoking accelerates the decline in ovarian reserve and worsens egg chromosomal quality. Stopping at least three months before treatment makes a measurable difference.
Consider Coenzyme Q10: Widely discussed for its potential role in mitochondrial function in eggs. Evidence is not conclusive, but many fertility specialists consider it reasonable for older patients. Always discuss supplements with your doctor first.
Manage chronic stress: Elevated cortisol disrupts the hormonal environment needed for a good ovarian response. Adequate sleep and light activity during treatment support a better physical baseline for your cycle.
