ICSI, or intracytoplasmic sperm injection, is IVF with one extra step added in the lab. Rather than leaving fertilisation to chance, an embryologist picks a single capable sperm and places it inside the egg directly. Because only one usable sperm is required per egg, ICSI succeeds in situations where natural fertilisation in a dish would fail.

When Standard IVF is Enough

When sperm quality is poor, ICSI lifts the fertilisation rate dramatically, to roughly 60 to 80 percent of injected eggs, well above what standard IVF manages with the same weak sperm. From that point on, outcomes hinge on the woman's age, egg quality, and the health of the uterus, not on which fertilisation method was chosen.
